Important Notice to Foreign Patients

Otsuka Dental Clinic provides dental checkups and treatments for foreign residents who live or commute to Shinkoiwa.

For the safety and benefit of our patients, we ask that only those who can communicate in Japanese make appointments for dental treatment. If you have difficulty communicating in Japanese, please bring someone who can speak Japanese with you when you visit the clinic.
If you are enrolled in national health insurance or social insurance, please be sure to bring your card-type "My Number Health Insurance Card" or "Certificate of Eligibility Information."

Health Insurance Cards will no longer be valid from Tuesday, December 2, 2025.

Due to changes in the insurance system, new issuance and reissue of the traditional paper health insurance cards will be suspended from December 2, 2024. From December 2, 2025, the general rule will be to switch to "My Number Health Insurance Cards," which use My Number cards as health insurance cards.
Those who do not use the My Number Health Insurance Card will be issued a "Certificate of Eligibility Information." The qualification confirmation certificate is a certificate that allows you to receive insured medical treatment at a medical institution, just like a traditional health insurance card.

Patients receiving medical insurance should prepare one of the following:

1) My Number Card:
Please bring your My Number Card registered as your health insurance card.

2) Certificate of Eligibility Information Card:
If you do not have a My Number health insurance card, you will be sent an "Certificate of Eligibility Information." Please be sure to bring your "Certificate of Eligibility Information Card." You cannot receive insured medical treatment with just the "Certificate of Eligibility Information Notice."
For matters regarding the issuance of the Certificate of Eligibility Information, please check the information from your medical insurance provider. If you have any questions, please contact your insurance provider.
